Architectural Control Committee form

 Description of Document

Referred to as the ACC Form, submit this form to the Architectural Control Committee for approval if you are making any changes to the exterior of your property.

What you will need to do

• Fill out the application, sign and date the second page.  
• Scan and email to Christina Tirado of M&M Properties and in the Subject line write ACC Form and your house address.  Christina@mmpm.us

When is an ACC form not necessary 

If you are making repairs to your home that does not alter the appearance, it is not necessary for you to submit the form.

Example: Repairing a damaged roof with like kind tiles or painting the house with the same colors.  If you are unsure, contact board@parkwoodv.com for confirmation.
